 Please note that I typically prefer to have a brief conversation with my clients prior to their initial appointment. If you are booking online, please read through my fee schedule and review the location of my practice. It may be helpful to complete the referral form so that I have additional information. Once I confirm your appointment, I will email various consent forms to complete online and in print form. Thank you!
